Which technique does the practice use?

Patients travel for this surgery, so the state a practice sits in matters less than what it does. The counts below are ours, taken from what each practice publishes.

Tumescent liposuction

Dilute anesthetic and saline are infiltrated into the tissue before removal. It is the most commonly named approach across the practices in this directory.

Power-assisted (PAL)

A vibrating cannula does more of the mechanical work, which can reduce surgeon fatigue on the large volumes lipedema removal often involves.

25 of the 73 listed US practices name it. See which ones.

Lymphatic-sparing

Technique and cannula direction chosen to avoid damaging lymphatic vessels. Practices that name this are signalling awareness of the lymphatic considerations that distinguish lipedema surgery from cosmetic liposuction.

35 of the 73 listed US practices name it. See which ones.

Water-assisted (WAL)

A pressurised water jet loosens fat before removal. Developed in Germany, where lipedema surgery has the longest track record.

22 of the 73 listed US practices name it. See which ones.

Ultrasound-assisted (VASER)

Ultrasound energy is used to break fat apart before it is suctioned. VASER is the brand name most practices use for it, and it is the technique patients most often arrive having already read about. Practices that offer it usually cite the fibrous quality of lipedema tissue as the reason.

16 of the 73 listed US practices name it. See which ones.

Laser-assisted

A laser fibre is passed through the tissue before or during removal. Practices naming it usually do so alongside another technique rather than on its own.

9 of the 73 listed US practices name it. See which ones.

Awake or local-only

Surgery under local anesthetic without general. Relevant if general anesthesia is a risk for you, and it changes how many sessions a full course takes.

Why does technique matter more than location?

Lipedema removal is high-volume work on tissue threaded with lymphatic vessels. Damaging those vessels can leave lasting swelling, so the approach a surgeon uses is a clinical decision rather than a preference. Most patients in this directory's data are choosing between practices in different states anyway, which makes technique the more useful first filter.

What is lymphatic-sparing technique in lipedema surgery?

Lipedema tissue is threaded with lymphatic vessels, and lipedema reduction removes fat from limbs where those vessels run. Lymphatic-sparing describes an approach built around not cutting them: cannulas moved along the long axis of the limb rather than across it, blunt rather than sharp tips, and infiltration that separates fat from surrounding structures before suction. It is a description of how a surgeon works, not a branded device, and no certification governs the term.

Is one lipedema surgery technique better than another?

No published trial establishes one technique as superior for lipedema, and the practices in this directory disagree with each other in print. What the literature does support is that lipedema surgery is different from cosmetic liposuction: larger volumes, fibrous tissue, and lymphatic structures that ordinary contouring does not have to preserve. A surgeon who can explain their approach in those terms is telling you more than the acronym does.

What should I ask a surgeon about technique?

Ask which technique they use for lipedema specifically rather than generally, how they protect lymphatic vessels, how many lipedema cases they do a month, and how many stages they anticipate for you. Whether they use one instrument or three matters less than whether the answer is specific.

Does naming a technique mean the practice is good at it?

No. These counts record what a practice publishes about itself, and this directory does not rank surgical skill. Use them to build a shortlist and to know what to ask on a consult call.

Water-assisted and power-assisted liposuction are frequently described as lymphatic-sparing by the practices that use them; the two labels answer different questions, one about the instrument and one about the approach. The fuller question list covers cost and aftercare too.
